Planning ahead for your family’s oral health care can be difficult when you are on a budget. When money is tight, it’s easy to put off routine dental checkups, professional teeth cleanings, and X-rays. But avoiding routine dental care is never a good idea because it can lead to far costlier problems later on, like cavities, root canals, or gum disease.

Drs. Gardner and Leiker believe every individual deserves good dental health, and that depends on routine dental care. To ensure it is within financial reach for all our patients, we offer low-cost dental care plans to our uninsured patients. To sign up, simply visit our website or contact our office. To be clear, this is NOT an insurance plan. It is more like a gym or health club membership. You can receive routine dental care for a healthier mouth for a monthly fee. This is a simple, monthly or annual subscription payment for preventive dental care. You can pay a low, monthly or yearly subscription payment for your preventive dental care, then schedule your appointments when they work for you.

Plan Disclosures
Our membership plans are provided only to uninsured patients of our practice and are not insurance, prepayment for future services, payment for access to discounted services, or a qualified health plan under the Affordable Care Act. Instead, our patients elect to make a monthly or annual subscription payment for preventive care rather than paying at the time of each service.
The membership plan is a direct agreement between the patient and our practice’s legal entity. Membership plan payments must be current so the patient can receive the benefits included with their membership plan.
Membership plan benefits may not be combined with any other offers or discounts. Payment for additional treatment provided by our practice to the patient is due at the time of service, and our membership plans do not cover procedures completed outside of our practice.
Our membership plans are valid for 12 months from the patient’s Effective Date and automatically renew on the Renewal Date. Patients can opt out of membership plan renewals by clicking “Cancel Auto-Renew” in the Member Portal or by contacting our practice directly. Refunds are at the sole discretion of our practice.
